YOGA CLASS WAIVER

I, the undersigned, hereby acknowledge and understand that participation in onsite/online yoga classes and related activities conducted by Shape-N-Sculpt involves certain risks and potential dangers. By signing this waiver, I voluntarily agree to participate in the class and assume all risks associated with my participation.

Assumption of Risk: I acknowledge that I am voluntarily participating in the yoga class(es) provided by the Provider. I understand that physical exercise, by its very nature, carries with it certain inherent risks, including but not limited to physical injury, strain, sprains, discomfort, and even the possibility of serious injury or death. I hereby assume all risks and responsibility for any such injuries or other medical incidents.

Waiver and Release: I hereby release, waive, discharge, and agree not to sue the Provider, its employees, representatives, affiliates, or agents from any claims, demands, liabilities, rights, damages, expenses, and causes of action of any nature arising out of or in connection with my participation in the yoga class(es), whether caused by the negligence of the Provider or otherwise.

Medical Representation: I represent that I am physically fit to participate in the yoga class(es) and have no medical condition that would prevent my safe participation. If I have any medical conditions or concerns, I have consulted with a healthcare provider and obtained clearance to participate.

Consent to Medical Treatment: I hereby consent to receive any necessary medical treatment resulting from my participation in the yoga class(es) and agree to bear all costs associated with such treatment.

Responsibility for Personal Property: I acknowledge that I am responsible for my personal property during online yoga classes and release Shape-Sculpt from any liability for loss or damage to personal belongings

Online Platform: I understand that online yoga classes will be conducted through a virtual platform, and I’m responsible for ensuring for necessary equipment, a stable internet connection, and a suitable space for participation. Shape-Sculpt is not responsible for any technical issues on my end. While Shape-Sculpt will strive to conduct yoga classes seamlessly on an online platform, some interruptions or errors could occur during the classes.

Class Rules:

·        Payment: Regardless of when I sign up during the month, I understand that I need to pay the full price for the session/membership/events.

·        Non-refundable and non-transferable: The classes are non-refundable and cannot be transferred to others.

·        Fixed time slots: Once enrolled, I cannot change class time slots.

·        Missed classes: If I miss a class, there won’t be a makeup session, a recorded video can be shared on request.

·        Confidentiality: I agree not to share the class Zoom link or materials with anyone else.

·        Zoom meeting name: I’ll join the Zoom meeting with my full registered name to avoid being sent to the waiting room.

·        Individual accounts: Each student must create their own account to sign the waiver and enroll. Sharing Zoom links with family and friends is prohibited, and non-registered members cannot participate in the classes.

Code of Conduct: I agree to behave professionally and respectfully towards others. Any form of harassment, discrimination, or inappropriate behavior will not be tolerated and may result in my immediate removal from the enrollment without refund.

Photography and Video Release: I hereby grant permission to Shape-N-Sculpt to take and use photographs and videos of me for promotional purposes without compensation. I also understand that audio or video recording the online yoga classes on student's end is prohibited.

Data Privacy: Shape-Sculpt do not sell, trade, or otherwise transfer my personal information to third parties without my consent, except as required by law or in the case of third-party service providers essential for the operation of online services.

Changes to Terms and Conditions: I understand that Shape-Sculpt may modify these terms and conditions time to time without notice. It is my responsibility to review these terms and conditions periodically for any changes.

Cancellation: I understand that Shape-Sculpt will try best to conduct the classes as per the schedule; however, classes may get canceled with or without prior notice in an emergency situation or sickness.

Acknowledgment:
I have read this Shape-N-sculpt yoga class waiver, understand its contents, and agree to be bound by its terms. I understand that I am giving up substantial legal rights by signing this document.
